Осложнения. Гнойные (возможны как при вирусном, так и при бактериальном ОТФ):






Гнойные (возможны как при вирусном, так и при бактериальном ОТФ):

- паратонзиллит/паратонзиллярный абсцесс

- заглоточный абсцесс (преимущественно, у детей до 6 лет)

- парафарингит / парафарингеальный абсцесс

- боковоглоточный абсцесс

- гнойный лимфаденит

 

Негнойные осложнения острого стрептококкового тонзиллофарингита:

- острая ревматическая лихорадка (через 2-3 недели после купирования симптомов)

- постстрептококковый гломерулонефрит (обычно на 8-10 день после начала заболевания)

- синдром стрептоккового токсического шока (обычно на 8-10 день после начала заболевания)

- постстрептококковый реактивный артрит

- синдром PANDAS (педиатрический аутоиммунный психоневрологический синдром, ассоциированный со стрептококком)

 

Немотивированная задержка восстановления трудоспособности, слабость, нестойкий субфебрилитет, артралгии, сердцебиение и нерезко повышенная СОЭ, сохраняющиеся после перенесенного тонзиллита, в сочетании с ростом титров противострептококковых антител (антистрептолизин-О, антистрептокиназа, антистрептогиалуронидаза, анти-ДНКаза В) могут свидетельствовать о дебюте острой ревматической лихорадки. Риск развития ревматической лихорадки существенно нарастает при контакте с ревматогенными штаммами БГСА, а также при наличии данного заболевания, в том числе ревматических пороков сердца, в анамнезе у пациента и/или членов его семьи.

 

 

Прогноз

Острый вирусный тонзиллофарингит как заболевание, склонное к саморазрешению у иммунокомпетентных лиц, в подавляющем большинстве случаев заканчивается самостоятельным выздоровлением независимо от проводимого лечения.

Благоприятный исход при остром стрептококковом тонзиллофарингите, который также может разрешиться самостоятельно, определяется отсутствием негнойных осложнений, что достигается назначением системной антибактериальной терапии. При этом задержка старта системной антибактериальной терапии на несколько дней (например, при ожидании результатов бактериологического исследования) не повышает риска развития острой ревматической лихорадки.

Особого внимания заслуживают пациенты с первичными и вторичными иммунодефицитами (в т.ч. ВИЧ-инфекцией), сахарным диабетом, что обусловлено относительно большим риском развития гнойно-некротических осложнений с фульминантным и, иногда, фатальным течением.
